K先生との最初の診察で自分がステージ４であると知ってまだふわふわ漠然としていた手術への決意が確固たるものになってゆきました。そこからは術式について悩んで悩んで悩みまくりました。ネット検索で術式についてはほぼ全ての種類を把握できたはず…便利な時代ですね。
いちばん引っかかっていたのは子宮をとるかとらないかという点でした。
小さな筋腫はあるものの癌などなく生理も順調でこれといって悪くない臓器を本当にとってしまって良いのだろうか。子宮を温存しておく方が良いのではないか。手術まで一年半待ちの某病院では、子宮温存したまま膣を縫い縮めるというマンチェスターという術式もあった。やっぱりそちらの病院を待つ方よいのでは….。しかし調べるにつれてマンチェスター術は再発のリスクが高く、ステージ４まで進行している今の私の状態では推奨されていないことも分かったのでした。
子宮を全摘する方法は大きく分けて３種類。
・膣式手術（アソコの穴からとる）と
・開腹手術（お腹を切って上からとる）と
・腹腔鏡手術（お腹を切らずに穴を開けて腹腔鏡で見ながらとる）
私の場合はかなり下に来ているので膣式全摘手術をK先生から勧められました。納得いくまで調べ尽くし、つまり子宮全摘が私には最適なのだ、という結論に達したのでした。

What bothered me the most was whether to remove my uterus or not. I have a small fibroid, but no cancer, my period is normal, and I’m not particularly suffering from any problems. Is it really okay to remove an organ like that? Wouldn’t it be better to preserve my uterus? At a certain hospital where I was waiting a year and a half for surgery, there was a surgical procedure called Manchester, which preserves the uterus and sews the vagina tighter. Maybe it would be better to wait for that hospital after all…? However, as I researched, I found out that the Manchester procedure has a high risk of recurrence and is not recommended for my current condition, which has progressed to stage 4. There are three ways to remove the uterus:
Vaginal hysterectomy (removed from the vagina),
Laparotomy (removed from above by cutting the abdomen),
Laparoscopic surgery (removed without cutting the abdomen by making a hole and viewing with a laparoscope).
In my case, the uterus was quite low, so Dr. K recommended a vaginal hysterectomy.After thoroughly investigating until I was satisfied. After thoroughly investigating until I was satisfied, In other words, I came to the conclusion that a total hysterectomy would be the best option for me.
